A simple bell icon on menubar for unread PM notification.

  - Now it updates hash table data when user with query open changes nick, then counter can be cleared properly later.
  - Start clean routine when query window are closed without taking focus, ie. from tree/switchbar context menu.
  - New Unicode char depending on Windows version, bell for Windows 8+ or balloon for Seven or bellow.
  - Changed to right aligned counter in bell menu for better organization.
  - Added command /clearbell to force remove bell icon and counter data, in case something goes wrong or whatever.
  - Other minor changes.
